You've cleaned it and done it properly too – you've used bleach, hot water, the lot. And ten days later you walk into the bathroom and there it is again, that faint sour smell you can't quite locate but definitely aren't imagining.
This is one of the more quietly maddening household problems, because it feels like a hygiene failure when it usually isn't. The problem is that the smell isn't coming from where they think, and cleaning alone was never going to fix it.
Here's what's actually happening.
The smell isn't the brush. It's what's underneath it.
When you put a wet brush back into its holder, water runs down the bristles and pools in the base. In a sealed container there's no airflow, no drainage and no light, so it sits there at room temperature with a small amount of organic residue in it.
Within a few days, bacteria in that water form biofilm - a slimy layer that anchors itself to the plastic or ceramic surface. Humidity that never lets it dry. In Brisbane, Darwin or Cairns through the wet season, a bathroom can sit above 70% relative humidity for weeks. A damp holder in that environment doesn't dry between uses - it just stays wet.
Bathrooms sealed up over winter. Down south the problem inverts. Window shut, door shut, exhaust fan running for the length of a shower and no longer. Moist air stays in the room, settles on cool surfaces, and the brush holder never gets a chance to dry out.

How to stop a toilet brush smelling: the prevention rules
Deep cleaning resets the problem, but it's these habits that keep it from coming back.
Never return a dripping brush. This is the highest-impact habit on the list and it costs you five seconds. After you flush and rinse, tap the brush firmly against the inside of the bowl several times, then rest the handle under the toilet seat with the head over the bowl so the brush can drip for a minute while you finish up. Less water going in means less water sitting there.
Empty the base weekly, not monthly. Biofilm takes several days to establish. A weekly tip-and-rinse interrupts it before it can anchor properly, and only takes twenty seconds. This is far more effective than a heroic monthly scrub.
Let it dry out completely, regularly. Once a week, drain the holder and leave it somewhere it can air properly - outside in the sun for an hour is ideal. Biofilm needs moisture continuously. Break that and you break the cycle.
Run the exhaust fan for twenty minutes after a shower. Most Australian bathroom fans are undersized for the room and switched off far too early. If yours is wired to the light switch, consider having an electrician fit a run-on timer - it's an inexpensive job and it improves everything in the room, including reducing the potential for mould on the ceiling.
Stop keeping disinfectant in the base. A lot of people keep a splash of bleach or Pine-O-Cleen sitting in the bottom, believing it keeps things sanitary. Anti-microbial material properties. Worth having, but keep expectations realistic. Anti-microbial surfaces slow bacterial establishment; they don't sterilise anything, and they're no substitute for drainage. Treat it as a supporting feature rather than the headline.
Genuine enclosure. A fully enclosed tube contains odour between uses instead of letting it circulate through the room. It also keeps the brush out of sight, which is worth something on its own.

What to be sceptical of: built-in air fresheners and scented inserts. They add a fragrance on top of the smell without targeting the source, and within a fortnight you have both. If a product's odour-control claim rests on a scent cartridge, it hasn't solved anything.
